For most women, breasts appear smaller, less full, and somewhat looser in shape after implants are removed, though the degree of change depends on individual factors. Why Do Breasts Change Shape After Implant Removal?

Implants stretch the skin and supporting tissue around them, and that stretching doesn’t necessarily reverse itself once the implant is gone. The amount of stretching can become more noticeable with larger implants or implants that have been in place for many years.

Once I remove the implant, there is less volume filling the stretched skin. This is why breasts may initially look flatter, looser, or more deflated rather than simply returning to their pre-augmentation appearance.

Will My Breasts Be Flat or Saggy After Implant Removal?

Some degree of flattening or sagging is common. Breasts typically appear less full at the top (upper pole), softer overall, and may sit slightly lower on the chest or look somewhat uneven. However, not everyone experiences significant sagging.

What Determines How My Breasts Will Look After Explant Surgery?

Three factors consistently drive the outcome of explant surgery: implant size, when your implants were placed, and the condition of your skin. Let’s dive deeper into these areas:

  • Implant size: Larger implants stretch tissue more, so the change after removal tends to be more pronounced.
  • Time with implants: The longer implants have been in place, the more the skin and tissue have adapted, and the harder it is for them to retract after removal.
  • Skin quality and elasticity: Skin with greater elasticity may retract more effectively after the implant is removed.

Will I Need a Lift After Breast Implant Removal?

Most of my patients do benefit from a breast lift performed in conjunction with explant surgery. A lift repositions breast tissue higher on the chest, removes excess, loose skin, and can relocate the nipple-areola complex for a more youthful contour. This approach effectively addresses the sagging and shape changes that implant removal alone can leave behind.

I don’t recommend a lift automatically for every patient; some women with smaller implants and resilient skin do well with removal alone. I make that call individually, based on your anatomy and goals, not a one-size-fits-all protocol.

What Are the Side Effects After Breast Implant Removal?

After explant surgery, it’s common to experience these side effects:

  • Temporary soreness
  • Some swelling and bruising along the incision sites
  • A sense of “unfamiliarity” as your chest adjusts to a different weight and shape

These effects are typically mild and manageable with prescribed or over-the-counter pain medication. Less common side effects can include temporary changes in nipple sensation and, if capsular contracture was present, some tightness as scar tissue is removed. Many patients describe the physical recovery as easier than their original breast augmentation procedure.

How Long Does Breast Implant Removal Recovery Take?

Recovery from implant removal alone typically takes about a week before returning to work, with light walking encouraged early on to support circulation. If a breast lift is performed at the same time, recovery involves a bit more downtime. For example, you’ll want to avoid lifting anything heavier than a gallon of milk for about a month and hold off on strenuous exercise for roughly 6 weeks. Most patients are back to their full routines within that 6-week window.

Does Breast Implant Removal Leave Scars?

In most cases, no new scarring is created. Implant removal typically reopens your original breast augmentation incisions, so you’re not left with additional marks. If a breast lift is performed at the same time, I may need to extend that incision along the breast crease up toward the nipple to allow for tissue repositioning and skin removal. This additional incision is a trade-off most patients find well worth it for the improved shape, and I’ll walk you through exactly what to expect for your case during consultation.

What If I’m Unhappy With How My Breasts Look After Removal?

If sagging, volume loss, or asymmetry remain a concern after explant surgery, your options include a breast lift, fat transfer to restore subtle volume, or, for some patients, replacement with smaller implants if they decide implant-free wasn’t the right fit after all.
